Support Coordination

Coordinating services to meet your goals

SensesWA have dedicated Support Coordinators that will work with you to better understand, implement and manage your plan.  Your support coordinator will ensure a range of relevant supports are used to assist you to:

  • Live independently
  • Access your community
  • Build and maintain relationships
  • and achieve your goals.

Please note that your NDIS plan must include Support Coordination to use this service.

What is Support Coordination?

Support coordination is a capacity building support which helps you get the most out of your NDIS plan.
Once your NDIS plan has been approved your Support Coordinator will work with you and give you the skills and confidence to navigate the NDIS in the future.

How do I get Support Coordination?

This can be discussed with your NDIS planner at the time of your planning meeting. If approved, Support Coordination must be included in your NDIS plan in order for you to access it.

Support Coordination is a set amount of allocated time within your NDIS plan.

It is good practice to allocate time:

  • at the start of your plan to map what supports you need throughout that plan
  • in the middle to review how supports and funds are progressing
  • and at the end to prepare for and attend your plan review.

How can a Support Coordinator help me?

Your Support Coordinator can help you to:

  • Understand and get the most out of your NDIS Plan
  • Connect with local providers who best suit your needs
  • Build skills to monitor your NDIS budget and funding expenditure
  • Prepare for your NDIS plan review.

What should I ask a Support Coordinator before signing up with them?

  1. How will you help me develop and achieve my goals?
  2. How do I get in touch with you if I have any questions?
  3. How will you help me choose between service providers?
  4. What kinds of supports have you connected people to in the past?
  5. Is there a notice period for ending a service agreement?

What's the difference between support coordination and specialist support coordination?

A Support Coordinator and a Specialist Support Coordinator are very similar however there are some key differences.

A Specialist Support Coordinator will be funded where there are additional high or complex needs in your situation and will be a qualified and experienced practitioner such as an Occupational Therapist, Psychologist or Social Worker.

Specialist support coordinators will support you to manage challenges in your support environment which may include health, education, or justice services. Specialist support coordination aims to reduce barriers to implementing or using your NDIS plan.
